Who needs to file a Notice to Owner in Florida?

Any subcontractor, sub-subcontractor, laborer, or material supplier who does not have a direct contract with the property owner. If a general contractor or another sub hired you rather than the owner directly, you need an NTO to protect your lien rights under Florida Statute 713.06.
What is the deadline to file?

Your Notice to Owner must be served no later than 45 days from the date you first furnished labor, services, or materials to the project. Missing this deadline means losing your lien rights entirely. There are no extensions and no exceptions under Florida law.
What does the $59 include?
