Property owner's full legal name and mailing address. General contractor's full legal name and mailing address. Surety company name and address (if a payment bond exists). Job site address and legal description. Your company name and address. Description of your work or materials. Name of the party who hired you.
Document preparation
Ensure the NTO follows the statutory form. Verify all required language is included. Double-check all names, addresses, and project details. Prepare a separate copy for each recipient.
Delivery
Send each copy via USPS Certified Mail. Request return receipts. Confirm the mailing date is within 45 days. Record tracking numbers.
